Monitoring, Assessing & Care of Patient in Restraints

1.           When a restraint is used there is an increased need for patient monitoring and assessment to assure patient safety, that the least restrictive methods are used, and that the restraint is discontinued as soon as possible.

2.           The patient's initial assessment drives an individualized plan of care, and the frequency of monitoring will be as follows:

a.            Other trained care team members may take an active role in collecting data and address attention to needs, i.e. toileting, fluid and nutritional needs as appropriate to their discipline.
