OCCURRENCE VARIANCE REPORT (OVR)

All individuals working at Broward Health must report all unsafe conditions, events, or practices to their supervisors for corrective action as soon as they are discovered.  An OVR must be filled out immediately whenever an incident or near-miss takes place that injured or could have injured a patient or visitor.  Risk Management and Protective Services work closely to investigate, track and trend O/Vs in order to prevent recurrences. 

The OVR is used in a non-punitive way to alert management to possible system breakdowns, quality of care issues and liability, and helps to improve system quality.  The O/V report is a prompt and accurate account of the details of an unusual incident and provides a method for root cause analysis, and prevention. The report must be received by Risk Management within three (3) days of knowledge of the incident.

Please fill in form, place in a CONFIDENTIAL envelope, and hand carry or place directly in mail slot to your Risk Manager.
